Services Agreement
This agreement for Mako Services, Inc. at _______________ is made as of ____________ by and between:

And Mako Services, Inc. (MSI in body of contract)

1. Services - Subject to the terms and conditions of this Agreement, MSI agrees to provide the necessary personnel and equipment to eliminate or relieve the Conflict (hereafter known as Case), as specifed by Client in Section 2 of this Agreement. Once this Agreement is finalized and the requested down payment made, MSI will complete the Case quickly and discreetly while minimizing any liabilities that may arise during or after its completion.
2. Case - MSI agrees to assign a MSI Agent with the purpose of eliminating/neutralizing (choose only one) ______________ (Full Name) permanently/temporarily (choose only one).

Reference Number issued to Case is ____________. This identifying number should be utilized whenever Case is referenced in order to ensure accuracy and confidentiality.
If a Case's solution is to be of a temporary nature, the fix should last _________, as specified by Client.
Each Case warrants its own separate contract. The manner and method in which each Case is handled is best determined by the MSI Agent assigned to it.
3. Payment and Terms - Client will pay to MSI the sum of $______ via cash or wired funds for services rendered. An initial nonrefundable payment of $_______ (50%) is due upon signing this agreement and the remainder is due upon completion of Case.
This fee does not include the travel expenses that may be incurred by the MSI Agent assigned to Case. Client agrees to provide at no cost to MSI all such costs, including but not limited to: hotel accommodations, meals, and transportation, such as cab fare, rental car, airfare, train fare, etc.
Failure to pay will result in the termination of Client.
4. Client Consent - Client hereby grants to MSI and its employees, agents, and independent contractors and suppliers unlimited permission to do what is needed to successfully, efficiently, and discreetly complete Case beginning on _______ and continuing until accomplished.
Client assumes all responsibility for any damages, both property and physical, incurred during Case and for any legal issues that may result from Case completion.
5. Release and Waiver - Client further agrees to release, indemnify, and hold harmless MSI, its employees, agents, contractors, and suppliers against any and all losses, accidents, damages, injuries, expenses, and claims resulting in whole or part, directly or indirectly, from Case.
Client understands that MSI can only guarantee the completion of Case. While MSI shall use its best professional efforts to achieve a successful result, MSI does not and cannot guarantee success or any particular consequence, resulting from Case completion.
This Agreement contains the entire agreement of the parties regarding Case, and there are no other promises or conditions in any other agreement whether oral or written, with the exception of the Confidentiality Agreement.
This agreement shall be governed by the laws of the State of New York.
